I have the latest version 1.0.3 My contact form is still missing. The above article does not help me because it is for shortcode. My shortcode tab is there.
Try to reinstall the plugin and clear wp-crm configuration in your database (in wp_options table).

Submit button exist by default. You do not need to add it. Try to deactivate all third party plugins in order to investigate the issue. You can also contact us directly using this form to troubleshoot the issue more directly.
