    Want to set up a Subscribe/Follow popup.
    Assuming a contact form must be included?
    The default contact form does not display in popup.

    How can a link be setup so potential subscriber can submit info and it will be sent to me???????

    It all depends on what service you use for subscribing users. It is mailchimp or something similar?
    For example our premium version includes optin forms that integrates with major marketing tools.
    Not sure which service you need.
