Constant Loading at widget placement

Hi there – when setting this up, I ran into the No List Set issue. I resolved that issue – however I noticed 2 things. There is a constant loading/refresh symbol next to the widget when viewing my site, and secondly when adding the populated short code into a pop up widget, I noticed it had the following within –
no_list_error=’– NO LIST HAS BEEN SET! –‘
Is this causing the constant refresh?

In addition to this – the shortcode when pasted into the popup is populating everything, but gives me an error when attempting to submit. It does work in the widget, but with the loading.
Another update – I removed the shortcode and used the html that was populated instead. This worked to create the form – however, the submit button does not work. This is in reference to the pop up – the widget on my sidebar works.

– code not working in another widget (sticky footer)Actually – I just decided to update the code from scratch and now they are both working. Not sure what changed – but I am good to go. Thanks!
