Have been able to replicate this issue and I am now looking into developing a fix. It may be an issue that I cannot solve and will need to get in contact with the Relevanssi developer. I think what needs to be done is ensure that Relevanssi doesn’t index pages with a quiz/survey on – will have a play and will see what I can come up with.
Edit:
As a temporary solution you can go into the settings page for Relevanssi and under indexing options change ‘What to Include’ to just posts. This will mean that your other pages are not searchable but this is more reasonable than it randomly chucking out the quizzes. I will get a proper fix sorted soon.

Yeah I’ve experienced that myself when I was testing.

Brutal, thanks for looking into it Ollie.
Any ideas since he isn’t able to help? I haven’t been able to specify which post_types to index with his plugin. When I try it returns absolutely nothing. So I end up just indexing everything.
Are we able to exclude_from_search on all quizzes?
I’ve added the exclude from search on all quizzes but I’m not sure how it works. He mentioned that it only works on posts rather than quizzes though. So until he adds the ability to exclude pages then I’m not really sure.
Haven’t looked at it for a little while but I think I can remember there being an option to exclude pages by page ID? Maybe have another look and see if that’s an option.
There isn’t really anything I can do on my end to fix this, as it’s an issue with the indexing and display of his plugin.
If I get some spare time then I will have a look at Relevanssi and see if I can add it myself. I will let you know if I get anywhere with it – might be worth periodically checking the thread I linked to see if he gets back to us.
Until then I can only suggest the temporary fix I mentioned earlier or just disabling Relevanssi all together. It’s up to you, whatever would work best for your website. In my opinion I can’t see much advantage to using Relevanssi though.
