I point out a potential conflict between the Forminator plugin and the Hueman theme. Here are some reports. Perhaps it is the case of a comparison between developers to understand the cause.

Sorry to hear you are having this issue.

Is it for the same signature feature?

If so, this is a Pro feature, please open a thread on our support section for Pro members https://premium.wpmudev.org/support-form/

I also found we provided a hotfix for the user of the reported link, if this is the same problem can you try to upload this plugin and see if the issue is gone? https://github.com/patrickfreitasdev/signature-fixer

In case it is a different problem with a Free feature, kindly let us know and we can try to replicate the issue.
