Can you please make a full screen screenshot of where you’ve activate the integration for the contact form so I can check those settings?

Comment Integration checkbox is not appearing on my site, contact form checkbox is appearing.

If you set Implicit to “yes” it means the option is not shown, and it will implicitly subscribe everyone that leaves a comment.
Set that setting to No to show the checkbox.

One more question I have. I have set up the double opt-in from the MP4WP plugin. Now, the email is coming from my old email id. Now, I am not having the setup where I need to go and update it to my most used email id. can you help here, please.

That email is send by MailChimp.com, not by our plugin, so this will be in the settings in your MailChimp account, under the audience settings I think. You can contact MailChimp for further support.
