Some browsers save information when you enter a form.
Use the settings of your browser to disable this.

Why do you need to remove the script below the comment box?
Sorry to trouble u. I want to disable but I dont know how to disable it. and I wanted to remove the word “sample page” button beside the “home” also. thank u so much agian. u r really my guru (guru mean teacher in india language) 🙂

If that screen shot is what you’re referring to, that’s the normal behavior for after you’ve left a comment as a non-logged in user.
The cookies tell the comment form your name, email, URL so you can continue to comment without typing that in again.
Nothing needs to be disabled on your installation. 😉
