CodeCanyon Reviewer Plugin Issue

I recently downloaded a plugin from Code Canyon called Reviewer.

I am trying to register it so that I can use it, but when I fill out all of the fields and click submit, I get an error that says, “The wordpress theme url field is required.” However, that field does not exist here.

Is this an author issue or a WordPress issue? I have contacted the author, but he has yet to respond.
