Clean and Simple Contact Form Plugin Help

Hi there, i’ve just downloaded

Clean and Simple Contact Form for WordPress but when i go to the contact us page on http://www.indotrek.co.uk they are some small boxes at the top just before the start of ‘Please enter your contact details…’

How do i remove these?

Also how do i go about changing the font to Arial and size to 14px, the same as the other pages on my site.

Any help would be greatful. thanks.
