Can you please check your browser console to see if there are any Javascript error being reported? If yes, can you please share a screenshot of the same?

Thank you for sharing your form export.
After reviewing your form and running some tests I found the issue, you have changed the default field colors but set the radio and checkbox icon color to white and that is why the checked items are not visible.
To fix this issue, you should go to form editor -> appearance -> colors -> radio and checkbox and change the icon color to anything other than white.

Could you please share the form again, but with the changes made, or publish it to a test page and share this page with us? So we can check it directly there, you can hide any sensitive info from the form or the page before sharting it here.

Thanks for the link. I checked the form by clicking the “Devis sur mesure”, but the button didn’t worked. I had to load the page without cache:

That made the button show a pop-up but the form doesn’t load. When I checked the code of the page, I saw that the Forminator form code is there, but the form has a “display: none” rule:

Forminator may add this CSS rule on purpose when it detects there is an issue with the site, and couldn’t load all the necessary assets from the site to work. Please check this documentation for more information:

I suggest you to make a backup of your site, and run a Conflict Test, in order to find which other component is causing this.

Good afternoon,
Nothing work.
After almost 1 month trying to find solution with forminator, I think I will abandon this plugin. I spent a lot of time solve issue, making me very late in all my work. First time I work on wordpress and cannot find a solution… Very disapointed about this pluggin and time lost ! Just frustation is on…

I understand that this issue is taking too long to resolve, but it’s not directly related to Forminator itself. The main problem stems from conflicts with other plugins installed on your site. For example, WP Optimize has certain features that can disrupt the functionality of other plugins.
I have reviewed your site again and I can see some important JavaScript files, like jQuery Core are being delayed; you need to exclude them from WP Optimize plugin optimization to fix this.
To test, you can temporarily disable the WP Optimize plugin to make sure that its causing issues.
I was able to fix the radio button issue in my lab site by changing the icon color as mentioned above. If you are still having issues with that, please reset the color customization.
