Checkbox HTML Not Working on Page

I have an excellent theme I’ve been working with but when I try to add checkbox or radio buttons within my HTML form, they don’t work!

Could it be possible this is somewhere disabled within my theme?

Here is my code:

<p><input type="checkbox" name="terms"> I accept the Terms</p>
