Change URL but keep Google ranking?

Hi guys,
I have an existing wordpress blog, self-installed with lots of posts comments. We want to change its url and possibly permalink structure to merge it with another site.

How can I do this without losing the precious google rankings on the old blog posts? I have found some info on 301 redirects and plugins but does it really work and can you suggest the best way to go about this?
