[cfdb-save-form-post] isn't working

Hello,

I am currently trying to get this shortcode to work with no luck.

The form is located at: http://amstrdmnyc.com/contacttest/

I have correctly put in the hidden fields, shortcode and making sure the form posts.

However in my CFDB page I have no record of any submissions from this form after I use this form.

Any suggestions?
