Capturing subscriber names

I want to capture both email addresses and names. I have the name field showing, along with a user prompt by using this coding:

<p>Enter your name,</p>
<input type=”text” name=”submitted” id=”submitted” value=”” />

However, when I tested the form, I only received the email address and not the name I inputted.

How can I force the plugin to email me the values from both fields?
