@nesoor I have tested the log in captcha feature in my test environment, and I cannot replicate the issue.
There are two possibilities:
1. You open the login form and wait 30 minutes, and then submit the login form. Math captcha is valid only for 30 minutes.
2. You are using any WordPress Optimization plugin that clears transients frequently. This Optimization plugin may clear transient between you to visit the login page and submit the login form.
Let me know what happens at your end.

Somehow it’s working again so what might have caused the issue I am not sure.
However, I did make a change in WP Rocket transient so that might be the cause. I just can’t tell for sure.

@nesoor We are discussing with my team member how can we remove this conflication with the Cache Plugin and We may be integrate the solution next release. We may add an admin notice when Simple Math Captcha is enabled and the cache plugin is active. I am sure that we will work on it, but how It is not decided now.
