Start disabling plugins. this is not a multisite restriction.
@andrea: OP orginally posted in the HowTo forum where we ruled out plugins. I suggested posting here when the OP mentioned he was using multisite. Since plugins, themes and corrupt core files have been ruled out & I’ve confirmed that the Editor role can post uncaptioned pics in a single install, that leaves only the server or something within the install itself.
Since multisite set ups are more complex (and not within my area of expertise yet), I wondered if you could come up with anything.

Nope. no restriction on this that I know of.
Tested on a site and it works as expected…
Did you test the default theme? (Knowing Esmi, I’m sure the answer is yes, but I’m CYAing!)

Just a thought but would it be worth creating a new admin user in one of the blogs and seeing if there’s any difference when trying to insert uncaptioned images?
Yeah, I made a second user (an editor) on my test.ipstenu.org site and was able to make a post with an image, no caption. Changed user to admin, again, no problems.
Double check that user is the admin of that particular blog, but even then, if they can make a post, they can add an image. Caption or not.
It seems almost as though something is causing the “Title” and “Caption” attributes to get mixed up; as the title is the only required item (other than the attachment itself) for an attachment.

I’ll try creating a new suer as suggested. Thansk for all your support, its good to see people that are actually interested in being helpful on the web rather than people just after an argument.
Sorry I forgot to mention that I had already tried plugins etc.
I do come up with some doozies!! 🙂
