Can't add any form elements

just installed this plug-in and I can’t seem to add any form elements via Edit Form. Clicking on Text|Text Box| etc under “add form element” does nothing at all.
Running on 3.4.2, with no other plug-ins activated. Same issue in both Chrome and Safari. I’ve tried deactivating and reactivating the plug in.
