You will not be able to complete software updates on your website while the site lock is locked. You should unlock everything before you complete update. You can read more about this at https://plugin.wpsitelockdown.com/
As far as the database you should log into phpmyadmin and search for the term “cialis” to find the option that contains this term. In some cases this might just be a term used by a spam catching plugin and could be a false positive and you can leave it as is.

Hi thanks yes I unlocked the wp content and didn’t work until I refreshed the page a few times. I am worried about the spam that it says in the database that I can’t find. It is a site that has translations for Chinese. It uses WPML and that is the item in the database, so what to do???

Please log in to phpMyAdmin and search the database for the term “cialis” to identify the option or entry that contains it.
Please note that, in some cases, this term may be associated with a spam-filtering or security plugin and could therefore be a false positive. If you confirm that the entry is legitimate and related to such a plugin, you can leave it unchanged.
If you are unable to find the term anywhere in the database, then it does not appear to exist in the database and no further action is needed.

OK thanks, so I will check once again a there was a compromise and someone rolled the site over to a new database of which I removed as it was not used anymore and same for the duplicated site etc. I could not find it in the database in the phpmyadmin search though google search still flags something. If I can’t find it the database, do I leave it saying that it is flagged forever? If not how to remove?

There is an option to ignore the flag if it is not malicious.
