There is calendar and form fields from your actual theme or some other plugin. Please contact support of that plugin or your theme relative to that issue.
If you want to use Booking Calendar, then you need to downlad it from here and install relative to this instruction.

Ok, I found this one, but I need a booking calendar for multiple customers who will be renting their places. This looks like it is designed to use only for one site, and one available booking. Can’t I make multiple contact forms?? If so how, and where is the shortcode?
