    The issue is simple, there is installed a plugin on your website to optimize it, and you have enabled in this plugin to option to minify the HTML code generated. This provokes the code with form’s structure be replaced by the constant:

    Please, go to the settings of the optimization plugin and disable the option that minifies the HTML code.
