I can add it as a block or the shortcode, both seems to work.

if it can be added by shortcode , then please try go to Cache -> ESI -> enable ESI, then convert the shortcode from [xxx] to [esi xxx ttl="0"] , see how it goes

I added shortcode to container. Enabled ESI and added [esi contact ttl=”0″] to ESI Nonces.
Is that correct? As well, options Cache Admin Bar and Cache Comment Form is OFF. Should I leave it like that? (see screenshot: https://ibb.co/zWN17sG2).
How do I check if only form is excluded, or can you see from your side’
thanks,

am confused, ha. I deleted previous and tried again.
1st – added [contact] shortcode to my contact page container.
2nd – went to Cache -> ESI -> enabled ESI.
3rd – went back to page with original shortcode and changed it to [esi contact ttl=”0″]
Is that what you meant?:)
Now Contact Form does not work (unable to send it).
