BUG : Two widgets instead of one

Hi,

I just installed the Yikes opt in form. Everything is working fine with it but it appears it has created a duplicate of itself.
The duplicate is right at the bottom of the homepage side bar and I can’t figure how to get rid of it.
In the widget settings only one opt in form appears.. it’s very strange.
