Bookking-my-account is empty

Hi,

The plugin works perfectly except the page booking-my-account: http://professeur-landie.fr/gestion-rdv .
The shortcode is well inserted.
In the front page, we find only a frame in the content and when we look at the html structure of the loaded page, I have this:
