Blank subscription popup

Opening a new thread as the suggested fix did not work. I have tried deactivating all other plugins and I’m still having the same issue where the pop-up is completely blank. No placeholder value in the text box and no “Submit” text on the button. Looking at the HTML it doesn’t look like anything is being passed to the value attribute for these two inputs. Not sure what else to try.
