I don’t have a caching plugin on my site unfortunately.
Here the list of active plugin on my website :
– GDPR cookie consent
– Maintenance
– Quill Forms
– Sidebar Widget by CodeLights
– Simple Universal Google Analytics
– Wordfence Security
– Yoast SEO
And the theme is “Divi”, right?

We have installed and activated all plugins mentioned + Divi theme and we can’t reproduce the issue. Is there any custom development done in your site or custom scripts?

Actually I just realized that it is not the mobile version that is not displayed! The form does not appear when I am not connected to the site and appears when I am connected

There is something at your website causing javascript files to be deferred and this doesn’t work in admin view. It works for any user or guest except for the admin. Might be a setting from your host?

We have noticed that you have a child theme of Divi.
Can you please confirm that there is no custom PHP code at this child theme that forces all scripts to be deferred?
If there is custom PHP code like this, this should be excluded at Quill Forms pages.
Please let us know and if there is any PHP code, you can paste it here and we will guide you for how to fix this.

you were right, it was a script on our side that we had put in a long time ago that caused the problem 🙂
Last question about the free offer: are there any restrictions on the answers to the form?

No restrictions in the free plan for answers. You can always use email notifications for free to have an email with the answers after each submission and this will be always free.
