Blank Page from W3TC

I am working for a client who has a custom plugin that shows programs for the user to register to. However recently the plugin is showing a White Screen of Death when trying to load the page from the plugin. After reading some of the articles on here I checked the html validity and also found some javascript that was included in the template etc. I worked on fixing that but the page is still being shown as blank. The page that is being called is coming from http://apistudyabroad.com/program-finder/ which is not being html minified.
