Blank Page

Hi all,

Some of my forms doesn’t appear when I add to a page example. Page is complitly empty and I tried everything.

Today I discovered another point. When I add the form code twice, ,it works. But it shows two times so I can use that.

Please help me about this problem, I am pretty desperate.
