As for the blank login it’s really hard to know what’s going on. It might be either a theme or plugin conflict.

I restored the default fields but nothing. and there is no script error in the console. If I look at the HTML the content of the page is all empty, there is nothing at all besides the title. I created a new page and put [usersultra_login] but it is the same, blank.
you should wait until we have replied to the post on our fourms. we cannot take care of the same issue on two different forums.
you will need to give us WP admin credentials on our forums so we can check the issue on real time.
