Automatically add automatic excerpts which include image?

The very first thing on most of my posts is an image, followed by text below the image. I want to the excerpts to automatically include the image plus a few text words.

I went into the index.php and changed the_content to the_excerpt, but this only includes text words, and it excludes the image.

Any way to include the image in all the post excerpts automatically?
