To achieve your goal you need to create two forms with the following specifications:
1. Create two new form for the homepage and “Get your cash offer” page
2. In the behavior tab of the first form add a new action as a redirect and add your “Get your cash offer” page with the name, address, and other contact info as parameters, here you can see an example:

3. Go to your second form and enable Pre-populate in each field settings then add the field name used on the URLs for redirect in the first form, for example, use the name for the name field (it’s optional but you can’t use the WordPress reserved names).

Thanks @wpmudev-support2. Any advice on how to make the address form into one line instead of having address, city, state, zip code on separate lines?

I’m afraid, there isn’t any such setting in the plugin side to change this behaviour. A workaround would be to consider using Input Fields instead of the Address field so that you can drag & drop the Input field in a single line according to your requirement.
If you are looking to change the Address field in a single line then it’ll require custom code.
