Ad Inserter does not stick or display anyting, it only inserts the codes you configure:

So you need to remove the code responsible for that ad.
If you are using AdSense Auto ads then this code may insert and display ads all around the page.
Remove the code for Auto ads or/and disable Auto Ads in your AdSense admin interface:

I think so as I include auto ads it appears nasty.
I not sure why the ad has appeared as it has, hasn’t done so before.
Really don’t want to deactivate the plugin, as it works great
It places a large skyscraper ad in the header which put the site out of shape.
Doesn’t matter if auto ad code or non-auto ads code, auto ads has been deactivated and once any Adsense code is placed adinserter, it automatically places this other ad in the header.
The code that places that ad needs to be identified.
As expained, the plugin only inserts the codes you configure.
