Attachment fields suddenly missing file input areas

My form on this link:
http://network-control.com /about/careers/ (<– space in link added intentionally)

…has no input areas for the attachment fields. I’m not sure what might have been the cause as this site auto updates plugins. The field requirement is the only detail I have checked on for each.

Any ideas about why they’re missing?
