annual renewal form

I have been trying to figure out a way to do this and have decided to toss it out here before my Head explodes. Every year I get a spreadsheet that list about 1500 people. It includes their contact information and some other data items. I export these to individualized pdf’s which get emailed to them (email is in spreadsheet) They make additions or corrections and mail back.

What I would like to do is create a web form that they can log into with a personalized link that is password protected (whether a one time code that comes from my spreadsheet or creating a subscriber account on my site{site is mostly open items so only admins currently have accounts}). They would fill out fields and checkboxes for any changes and save. This would come back to me in some form (emails, or csv).

Extra points if form is locked or unviewable after they save it.

Please help 2020 is tough enough on it’s own.
