Seems that You’re working on the regular WP authentication option, so I can’t see the problem.
I have this problem as well. Not always, only sometimes. Can’t find what is the reason for it mostly working and sometimes not.

I have the same issue from time to time too.

Just discovered it’s an issue with WP Fastest Cache plugin – as soon as I cleared the cache, the login worked again.
Mine cured after moving to other server.
