Following from my post above, on further investigation, it seems that this is happening only if I access that page over regular “http”.
If I access that page over “https”, this issue does not arise.
Any ideas…???

Hi, I cannot see the upload form either because I do not have any of the uploadrole roles. Can you please enable guests so that I can see the form?

I have added “guests” and now it works for both “http” and “https”.
As soon as I take “guests” off, it no longer works for any logged in member for “http”.

Now for the why: The internet is a wonderful place full of very nice people and a few very bad ones. Likewise the person who takes the keys is now responsible for the house FOREVER.
If something was to go wrong, then you the author may well legally become liable for damages, which they would not normally have been as their software is provided without warranty.
Please be aware that repeatedly asking for credentials will result in us asking you to repeatedly stop before escalating up to the plugins team.
