A request plugin

I need a WordPress plugin that makes these things:
1. The client can complete a form.
• Easy to access the form for signed users or non-signed.
• The user can able to become a member.
• Data form to be import in the database.
2. The form requires a service request or project and admin can develop a from request – a process in 10 steps, for example…
3. To every step process admin can write a note and if he wants an email client. A predefined email or a custom one.
4. The customer to be able to check the process on the website.

I think can be like: a Tiket system, issue report, or projects management.
