    Hi, I dont know why this is happening and I searched but couldn’t find any relevant information. I’ll try and describe what I did as best as I can.

    – I created a form about 6 months ago that worked perfectly. Quite a long involved form with conditions and different languages and such. Everything worked fine and the last successful form submitted and received was on 09-07-2013.

    – When a client notified me he couldnt fill in the form I went to the page only to find a blank page.

    – I first de-activated all plugins I installed since around 09-07-2013. Didnt change anything. Still a blank page.

    – I tried to play around by using shorter forms. This worked and the shorter forms showed up.

    – Then I started building a new form to look exactly like the previous one. I started using one of the subforms and just inserted it. All was fine, page showed.

    – I added fields one by one and tested. All went well and the page published.

    – When I added a second email field, and tested, the page went blank again. I deleted the Email field, tested, page showed fine. Added the email field again, tested again, page was blank again. Deleted it again, etc. I did this a few times to see if I missed anything.

    – For the life of me I cannot figure out how the field is a simple text field BUT just adding the name “E-mail” to it somehow broke the form completely.

    I hope this information helps and someone can replicate the problem and have a solution.

    I should have mentioned I used WP 3.5.2 and Form Manager 1.6.41.

    It seems that the problem has nothing to do with the email fields but rather that i can only add 20 Form Elements to a form.

    This seems like the kind of number that can be set somewhere. Does such a limitation exist in the script somewhere?

    Hi, I found out my service provider upgraded their servers to the new PHP 5.4.4

    I had the option of changing this back to PHP 5.3.3. This brought back all functionality to my form.

    I dont know how long my service provider will allow this to work with 5.3.3.

    Just so you know. Thanks for a great plugin.

    Hi there, my service provider changes over to PHP 5.4.4 permanently now.

    The form does not display at all. Its broken now and I rely on this plugin a lot.

    Do others have the problem with PHP 5.4.4 and the form not working?
