I’ve tried with no success. Registration and restore password forms both work fine, but no the login form. Maybe it is something related to an incompatibility with a certain plugin, could you please disable others (one by one preferably) and check it again? it’s quite confusing. I hope that helps.

Forget my comment above. That’s my fault!
I implement Google Captcha (reCAPTCHA) by BestWebSoft to change your default captcha, but i forgot to setting it.The login form work fine!
Btw, of my point:
– If you dont mind, please change your captcha with Google reCaptcha?
– Icon or background field not appear. Can you replace with Font Awesome Icons?

It’s already updated with Google re-Captcha thanks to an external contributor (thanks to @pablomargareto). I hope you find it useful.
Font Awesome is not listed because is not the default one in WordPress. Maybe you can review your style.css under your theme, in case it is overwriting something there.
