Yes, we do not have it activated on our web due to this issue. However we have a staging area where we have it. Maybe you can check it in there?

I do understand. OK, I just activated the plugin in our live web (https://www.ethosdata.com/). You can see the problem at the bottom of the web were appears one out our forms; which actually is the one that appears as popup when clicking on “Request a proposal” blue button on the top.

I can see the issue on your website, let me check what could cause this.
So here is what I can see currently:
#1 My GTM plugin is active, container is loading properly
#2 Inside your GTM container I can see two Facebook Pixels: one for pageview tracking and one for Lead event tracking.
#3 A LinkedIn Tag has been also added
#4 I do not see any GA tracking inside your GTM container
#5 You are also using a Facebook Pixel plugin, causing your Facebook Pixel to fire twice: once inside your website and inside your GTM container
I do not see however any reason why activating my plugin causes this form to show up in such a way.
Could you try creating a new GTM container leaving it empty and adding the new container ID into my plugin? If this resolves the issues, it means that there is something in your original GTM container. In that case you should copy tags and settings one by one to find out which tag could cause this issue

I have done what you suggested by creating a new GTM empty and the issue does not happen. Then I added the tags one by one and discovered that adding ay of the two tag of Facebook, the issue happens. Having only the one of LinkedIn, the issue does not appear; only as soon as I add any of the two tags of Facebook.

Since you have Facebook Pixels in your site using another plugin, you may omit Facebook tags inside your GTM. But the best case would be if you could migrate all tracking codes (including HotJar as well and GA of course) into GTM.
