Hierarchy display not working properly

Hello. I am using the plugin to display a sitemap of pages only, and some of my pages are not being properly displayed in the parent/child relationships. When I first setup the plugin, it displayed child pages as sub-list items of the parent page. Those older pages are still displaying as sub-lists, but more recent pages are displaying as a single list with no indents for child pages.

I see several others have brought up issues with hierarchical display. Any ideas on how to fix this?
