I hope all is well!
I’ve tested the Appointments on my site and can’t replicate the same. First, please make sure that the Send Confirmation email to “Yes” from Appointments > Settings > General.

Please enable “Log Sent email Records” from Appointments > Settings > General under the Notification Settings:

Try to book an appointment and check the Appointments > Settings > Logs for sent emails.
Note, when confirming pending appointments, check the “(Re)send confirmation email” option to send the notification email.

Would you please make sure that there is no conflict with another plugin?
Deactivate all the plugins, leave only the Appointments+ activated. Book an appointment and check if the notification email is sent.
If everything works, activate one plugin at a time to find the one that might be causing this.

Thank you for sharing a link to your site. I’ve troubleshot it a little bit further and it seems like the appointments are created with no service provider assigned.
Please create a dummy service provider from Appointments > Settings > Service Providers and assign a service to this provider:

On the appointments page instead of [app_schedule] use the [app_schedule worker="15"] where 15 is a user ID of the service provider.
Once the appointment is assigned t a service provider the confirmation email should be sent.

Hello eeverest,
Please remove all the dots “…” from your shortcodes. They should not be there.
Then please try this: to Appointments -> Settings -> General tab and assign dummy service provider to real user.

Please see there that dummy provider cannot be the same as user you are selecting in “Assign dummy service providers to”.
I would also you to check if you are getting any other WordPress emails – like password reset email.

The plugin sends from wordpress@site.com?
It is WordPress who sends emails – and that is default address that is used.
You can change that by using SMTP plugin
– https://srd.wordpress.org/plugins/wp-mail-smtp/
– https://srd.wordpress.org/plugins/easy-wp-smtp/
