No custom fields!

    Wow, I am appalled that this plugin does not allow for custom form fields. The only fields available are the standard name, email, etc. I just assumed that this plugin had that basic functionality, and now I’ve shelled out ~$30 for two upgrades and it turns out I can’t even have custom form fields. VERY disappointed.
