Plugin breaking down contact page

Hi,

The plugin works as intended, but it breaks down contact page on our website. When this plugin is disabled, the contact page works fine.

On our website, the contact page is loaded via a shortcod > [contact]

When this plugin is active and we call our contact page, the shortcode loads up and then the fields disappear all together. This is very weird.

Can you guys fix your code please?
