They discovered that something on their site, not related to Formidable, had interfered with all posts, pages, and form actions on their site.
In the past, we have only seen this issue caused by other plugins modifying posts on your site. A form action is a custom post type so if another plugin modifies post data, such as the menu_order of a post, it can break your form actions.
