Inner pages broken..

I have read and the documentation page at http://enigmaplugins.com/inner-pages-broken/ but I cannot find the proper codes to enter in the fields :
“Inner Template Header” and in “Inner Template Footer”

ps: I’ve already tried the following code for the Inner header but no result :
