JetPack Contact Form Switching Fields

Hello:
I have the form on two pages. On one is fine and unchanged.

On the other the form is constantly switching the “Telephone” field and the “In your words…” field to:

“Website” and “Message”

Cannot explain it. Cannot solve it. Please let me know what additional info you need from me to help solve this mystery. Thanks!
