At this time, forms can be added to posts, custom post types and pages. The sidebar option is planned for the next version.
You can easily delete the “No Spam Guarantee” disclaimer by placing in that field.

I placed  : in that field and it works fine. But actually I wanted to completely remove that box and get widther the email field.

For the time being, you can edit the following file:
wp-content/plugins/optin-forms/css/optinforms.css
Simply paste the following lines at the very end of the file (I assume you’re using form design 02):
#optinforms-form2-disclaimer-container
{display:none!important;}
#optinforms-form2-email-field-container {width:48%!important;}
and upload your new file to wp-content/plugins/optin-forms/css directory
