    No ideas, sorry.
    Perhaps an incompatibility between plugin ?

    I deactivated all plugins and it still fails to activate. It works on other sites. Could something be in the .htaccess causing this?

    I dont think so.
    May be a corrupted file in WordPress, reinstall WordPress again to see if it work !?
