Plugin Breaking Site

I’m using Version 2.40.3 with WP 3.5.2 and when I visit http://www.antiochbaptist-mtairyga.net/ with the plugin activated, I get these errors at the top of the page and a 404 error:

WordPress database error: [MySQL server has gone away]

The plugin has been working fine for a long time and seems to have broken just yesterday or today. I know this because I use InfiniteWP to scan all my sites daily.

The plugin is currently deactivated so visitors can see the site.
