Insert on pages (?) option not working

I have a multisite directory install of WP and using Code Insert Manager (Q2W3 Inc Manager)for over one year. It was working fine. But for about one month it stopped working. I tried to get it worked after a clean WP install. Still the error is persisting.
I could not find any alternative to this wonderful plugin. Also I checked the existing support queries, I could not find answer to my following problem. I need it to get work as quickly as possible.

Error-I: When I try to use following option, Insert on pages (?)[…], it returns a blank pop up window with a title select pages. No pages or posts are displayed.

Error-II: When I try to modify and save the entries from previous install, it returns a 404 error.

Please advise.

Hello!

Check your site/web server for security patches/plugins.
Looks like something is blocking access to plugin files which runs outside wordpress.

Thank you for the great support. In response to the clues provided by MaxBond, I contacted technical support of my webhost. They advised me as follows:

Could you please try disabling mod_security on your account using the ‘Web Security’ option in your cPanel, then allow 10 minutes or so for the change to take full affect.
==============
I disabled the mod_security and my problem got resolved.
Now I am able to have full functionality of this matchless plugin.

My mod_security is not on and I still get the blank page.

Thanks for the help but what I did was on my server I changed the php version from 5.4.15 back to 5.3.25 and everything is working now. I guess 5.4.15 just isn’t stable yet.
